Referring a Child to Little Lights Liverpool
If you feel a child may benefit from our care, please complete the referral form below. Our team will review the information and be in touch to discuss the next steps.
Referral FormWe are an inclusive and caring organisation and welcome families from all backgrounds. If you’d like to talk to us before making a referral, we’re always happy to help.
Please have the following information available where possible:
- Child’s details
- Parent or guardian contact information
- Medical diagnosis and relevant clinical information
- Details of the professionals currently involved in the child’s care
- Any additional information that will help us understand the family’s needs
If you don’t have all of this information, please complete as much of the form as you can.
Need advice?
The service is offered to babies, children and young people from birth – 6 years with complex medical needs. We specialise in respite, family support, as well as symptom management. Referrals can be made by a family member or a healthcare professional involved with baby, or child.
Who can access our hospice services?
At Little Lights baby hospice, we support babies, and children living with serious and complex health conditions, as well as their families. Our care is available for those who have a confirmed life-limiting or life-threatening condition.
A baby, or child may be eligible for support if they have:
- A life-threatening condition
- A condition where treatments may be available to help manage or prolong life, but there remains a risk that the condition may become more serious over time. Some children may need ongoing specialist treatments or intensive medical support.
- A life-shortening condition
- A condition that is expected to shorten a child’s life because there is currently no cure, or because the condition is progressive and likely to worsen over time.
- Complex medical needs
- Children with highly complex health needs who may require specialist equipment, ongoing medical care, or support with multiple aspects of their health and wellbeing. These children are often more vulnerable to serious health complications and may benefit from additional family support.
